Complete your Health History Form* Students must complete the Health History Form, enter up-to-date immunization dates, and upload immunization records in their patient portal by the deadline date. International Students: Obtain your Form I-20 and F-1 Visa* Sign into the OISS portal and complete the International Student Data (ISD) Form to receive your Form I-20 required to schedule your F-1 Visa appointment at the US Embassy / Consulate.	Immediate submission recommended due to visa delays. Questions regarding the Student Data Form (ISD)? Email the Office of International Student Services or contact them at 310.506.4246. To meet this requirement, students must select one of two options: 1) Enroll in SHIP or 2) Waive SHIP on the AHP Insurance Dashboard. *The requirement for US-based health insurance does not apply to international students studying exclusively online in their own country.
