APPEAL REQUEST FORM FOR CURRENTLY ENROLLED STUDENTS:

This form is to be used by currently enrolled students only, and only if making an appeal for the indicated academic year. Please return the completed form to the Office of Financial Assistance.

FEDERAL TITLE IV AID AUTHORIZATION FORM:

If you have federal aid, including a Pell Grant, SEOG Grant, Perkins Loan, Stafford Loan and/or a parent PLUS loan, and you withdraw from the University, you must complete this form. Please return the completed form to the Office of Financial Assistance within two weeks of your withdrawal date.

Title IV Authorization Form


INCOME AND EXPENSE STATEMENT:

This form is for the parents of dependent students who will not file a tax return or who has an adjusted gross income plus Worksheet A plus Worksheet B (from FAFSA) of less than $25,000. It is also for independent students with no income.

REQUEST FOR TRANSCRIPT OF TAX RETURN:

This is IRS Form 4506-T. If you need to request a transcript of a federal tax return, you can print this form, complete it and send it to the Internal Revenue Service Please include the student's full name and ID # under line 5 (example: Re: John Joe Doe #100000001).
