International Student Data Form

The International Student Data Form must be completed in full, to the extent it is applicable. Applicants who have applied for lawful permanent residence (or "green card") but have not been issued the I-551 stamp or document granting lawful permanent resident status may provide further information on this form in order to determine if a Form I-20 may need to be issued by Pepperdine University.

If you are attaching additional documents, such as bank statements or immigration documents, please write your name and date of birth on each document. Documents that cannot be matched with the proper applicant may be discarded.

Please download and print the form, have it signed by your sponsor and a bank official, and send it, along with your other admission requirements, to the appropriate office of the school at Pepperdine University to which you are applying for admission.

If you are unable to print this form, you may provide the same information on a separate sheet of paper that clearly states the your name and date of birth at the top of the document. If a bank official is unable to provide a certification on the form, a separate certification signed by a bank official, a bank statement, or other documentary evidence of financial support may be submitted.
